At San Diego PACE, we provide all care and services covered by Medicare and Medicaid (Medi-Cal in California) as well as additional services to help maintain the health and well-being of our participants with the goal of helping them live independently in their home and prevent the need for skilled nursing care. San Diego PACE offers easy access to care and closely monitors the health care needs of each participant which results in fewer health crises, ER visits, and hospitalizations, better management of medications, and improved quality of life.
Primary Medical Care and Specialties
San Diego PACE offers a holistic approach to care, we believe that treating the whole person rather than a symptom is vital. Primary Care Physicians (PCP) provide the primary health care on site at the PACE Clinic and necessary referrals are made to appropriate health specialists in a variety of fields for any additional care needed, including:

In many cases, personal care assistance in the home is an essential part of retaining independence at home. San Diego PACE offers skilled home health and/or home care for participants who meet the need for these services as determined by the Interdisciplinary Team.
Our Day Center is designed to offer the care and support services you need in one convenient location. Seniors who attend the day center also have an opportunity to participate in recreational activities, eat healthy meals, and develop new friendships and companionship. In addition, trained and specialized staff provide nutritional counseling, physical therapy, occupational therapy, exercise, and more. Our goal is to offer comprehensive care and activities that help you stay healthy and active and develop a strong sense of community.
Recognizing the multiple needs in the communities we serve, San Diego PACE has moved towards a fully integrated care model in order to provide supportive services beyond the scope of traditional primary care. Our integrated care model allows both primary care and behavioral health to work together.
The San Diego PACE Behavioral Health team works closely with the PCP (Primary Care Provider) to address a variety of concerns including depression, anxiety, sleep, substance use, etc.
Prescription drugs are prescribed and provided by the San Diego PACE PCP (Primary Care Provider) and at no cost to the participant. The San Diego PACE Pharmacist reviews prescriptions prescribed by all providers including specialists to ensure appropriate, safe and timely medication therapy for our participants.
Needed laboratory, diagnostic and x-ray services, and durable medical equipment and supplies are provided as determined by the IDT (Inter-Disciplinary Team).
San Diego PACE offers safe and efficient transportation services at no cost to ensure access to all needed services. Transportation services assure safe door-to-door travel to and from the San Diego PACE Adult Day Center and PACE Clinic, as well as to and from off-site specialty medical appointments. Transportation vehicles are thoroughly equipped for safety, with special accommodations for passengers with disabilities.
San Diego PACE provides Rehabilitation Therapy to help participants remain independent and able to continue living safely in their homes. Comprehensive rehabilitation and skill-building therapies are provided on site at the San Diego PACE Adult Day Center and include the following:
Physical Therapy— therapeutic exercise and fitness training
Occupational Therapy— making the home environment safe to use, assistance with activities of daily living, and improved self-sufficiency
Speech Therapy— speech recovery and improvement, swallowing, and improved cognition
In conjunction with the PCP, the San Diego PACE Nursing Team (RNs) specializing in geriatric care provides personalized preventative healthcare services and additional clinical support. RNs also assist with appropriate triage of participants and hospitalization follow up.
San Diego PACE Registered Dietitians (RD’s) know how vital a healthy and nutritious diet is to the health and well-being of each participant. San Diego PACE offers personally tailored and monitored meal planning and special diets that support the nutritional needs of each participant..
San Diego PACE Social Work team provides sensitive and caring support that addresses the emotional, family, and social needs of each participant.
