In April of 2010, San Ysidro Health opened the Adult Day Health Center with the vision of providing a place for seniors in the community to receive care as well as offer a place for socialization and activities. With the growth of an aging population in San Diego county and San Ysidro Health’s goal to expand senior services, the organization looked into the PACE program as another option to offer seniors.
In 2014, an application was submitted to the Centers for Medicare & Medicaid Services (CMS) and San Ysidro Health received approval. San Diego PACE opened its doors to the South Bay community in April 2015 after transitioning from the Adult Day Health Center model of care. Since opening our first center, we have expanded throughout the county to serve South, East, Central, and North San Diego. Our operation has enabled thousands of seniors to receive much-needed care and support to stay in their communities.
San Diego PACE is focused on providing the highest quality of care for our seniors. Through our years of experience, we are able to identify the needs in the communities we serve. Our participant population is very diverse and are mostly concentrated in the Southern region, however, knowing that there is a huge opportunity for growth throughout San Diego County, we are continually expanding our services to reach those who are in most need.
Our team of Geriatric Specialists is equipped to manage the care of our most frail seniors. Each participant is assessed by an Interdisciplinary Team (IDT) and a care plan is created for them. Our goal is to provide all around care for our participants in order for them to live well and thrive in their community.
San Diego PACE’s state-of-the-art facilities proudly serve 114 zip codes throughout San Diego. We encompass and provide life-changing medical care to the County’s South, Central, East, and North regions. PACE programs are funded by both Medicare and Medicaid (Medi-Cal) or private payment.
In PACE there are no out of pocket expenses if you have or are eligible for Medicaid (Medi-Cal), or are dually covered or eligible for both Medicare and Medicaid (Medi-Cal).
Participants who do not qualify for Medicaid (Medi-Cal) have a monthly premium.
There are no deductibles or copays for any medication or health care services approved by our care team.
Medi-Cal is California’s Medicaid healthcare program. This program pays for various medical services for children and adults with limited income and resources. Medi-Cal is supported by federal and state taxes. You can apply for Medi-Cal benefits regardless of sex, race, religion, color, national origin, sexual orientation, marital status, age, disability, or veteran status. San Diego PACE’s Eligibility Team can help you with your Medi-Cal application, change of county code, or help with your renewal if necessary.
PACE provides free transportation to and from any of our center locations, as well as, to other approved appointments to specialists or other services that is included in your care plan.
