Diabetes Prevention Program

Welcome to San Ysidro Health’s one-year, lifestyle-change program, Small Steps, Big Changes

In this program, patients work together in small groups to find successful ways to improve their eating habits and physical activity levels. This program has been proven to reduce the risk of developing type 2 diabetes. Most patients who complete the program lose 5 to 7% of their initial weight!

Who is this program for?

Our program is for patients who are diagnosed with prediabetes, have a BMI of 25 to 35, and are 18 years of age or older. If you are interested, but unsure if you qualify, please get in touch and we can help. We have programs for everyone!

How is the program delivered? 

The majority of classes are delivered virtually, however patients do come into the clinic for an in-person check-in four times during the year.

The frequency of the classes also varies. See below for a schedule overview.

  • Months 1-4: classes are once per week
  • Months 5-6: classes are every other week
  • Months 7-12: classes are once per month
What topics are covered in the classes?

Our qualified lifestyle-coaches will work with you to increase your knowledge and understanding of diabetes and give you practical skills to build healthy habits. 

Also, by working in a group setting throughout the one-year program, you will receive support and advice when you hit hard times. The group will help ensure you can get back on track, so that you achieve your goals.

Here is an outline of the topics that are covered in detail throughout the year:

  • Increase knowledge of type 2 diabetes, carbohydrates, and the fundamentals of healthy weight loss
  • Physical Activity and nutrition – realistic ways to incorporate healthy habits into your daily life
  • Goal setting, and how to get back on track if you stray from your plan
  • Staying motivated and overcoming barriers
  • Understanding and coping with triggers of unhealthy behavior
  • Stress management
  • Importance of sleep
