SYHealth is upgrading our computer systems to provide better care and an improved experience for you. We appreciate your patience as you may notice delays at our call center or during your clinic visits.
Originally established along the U.S. / Mexico border in 1969, the organization was founded by a group of mothers in search of medical services for their children. Over 50 years later, San Ysidro Health now provides innovative care to hundreds of thousands of lives in our care through a vast and integrated network of 50 program sites across San Diego County including:
San Ysidro Health and its program sites are designed under the concept of the Patient Centered Medical Home (PCMH) model of care, where the patient, not the support staff or provider, is the focus of all care and services.
Our organization’s name and brand identity pay homage to its humble beginnings in the community where it was established 55 years ago. This community was named in honor of San Isidro Labrador, the patron saint of farmers, day laborers, and rural communities—the working poor. These values of compassion and care for others still ring true in the organization’s mission to improve the health and well-being of all in the communities we serve, especially the most vulnerable across San Diego County.
The San Ysidro Health Leadership team is comprised of executives and board members committed to the goal of making quality healthcare accessible to thousands of patients through a vast and integrated network of sites across San Diego County.
San Ysidro Health continues to provide compassionate care in the communities we serve across the County. Read patient stories and see how we are making a difference every day.
San Ysidro Health continues building and strengthening a culture of diversity, equity, inclusion, and belonging (DEIB) that is intricately connected to our mission achievement. Our endless DEIB effort is the workforce catalyzer that makes San Ysidro Health a better place to receive health care, a better partner to the communities we serve, and a better place to work.
San Ysidro Health patients enjoy access to comprehensive medical, dental, and behavioral health services, as well as support services and programs that help with other needs like food, housing, and health education. Our providers and care teams are committed to producing the best possible results for patients, while giving them the tools to take an active role in their health and well-being. We are proud that over 145,000 people across San Diego County have chosen San Ysidro Health as their medical home.
In 2023, San Ysidro Health won several healthcare quality awards that recognize our commitment to high-quality, accessible care and celebrate improvements made in the health and well-being of our patients. These awards include the Health Center Quality Leader Badge which honors San Ysidro Health as a top performer, nationally. These awards help highlight that patient-centered, compassionate care can truly make a difference!
At San Ysidro Health, we track the performance of each of our clinics to make sure we are meeting our goals. By focusing on these quality measurements, we can celebrate patient progress and make improvements when they are needed.
